Thrive Care Partners is an important part of Hampton Roads ensuring that residents get the home care they need.
At Thrive Care Partners we offer a range of home care services. We can customize a plan to suit your individual needs or you can select one of our Pre-built Personal Care Plans
Home care (non-medical)
We can provide service in an ale carte form or we have pre-built plans available.
Our Individual Services are as follows:
- Respite Care
- Companion Care
- Light housekeeping duties
- Laundry & Linen
- Medication Reminders
- Transportation to & from medical appointments
- Transportation for personal errands
Pre-Built Monthly Plan Options
We have put together 2 plan options for you.
We have a Basic Plan & an Advanced Plan. We will complete an assessment on interviewing/initial visit to potential residents and will provide you with our opinion on which option is best, but you are always welcome to discuss options.
Basic Personal Care Plan is 20 hours monthly. Good for those that are still very independent but need just a bit of help with day to day tasks.
Advanced Personal Care Plan is 80 hours monthly. Good for those that need a more hands-on approach with most or all daily tasks, laundry and housekeeping duties.
We can also custom tailor a plan for your needs.
Payment Options
Currently, we offer only Private Pay. We can set-up monthly, quarterly, yearly. We can also tailor needs to purchase individual hours. Unused hours would bank and roll over to the next month/invoice cycle.
We are working with state organizations to be able to accept Medicaid Waivers & private insurances in the future.
